The individual voluntarily reports having a disability or prior disability based on the following definition of disability developed by the United States Department of Labor: You are considered to have a disability if you have a physical or mental impairment or medical condition that substantially limits a major life activity, or if you have a history or record of such impairment or medical condition. Examples of disabilities include, but are not limited to, autism, blindness, bipolar disorder, cancer, cerebral palsy, deafness, diabetes, epilepsy, HIV/AIDS, impairments requiring the use of a wheelchair, intellectual disability, major depression, missing limbs or partially missing limbs, muscular dystrophy, multiple sclerosis (MS), obsessive compulsive disorder, or post-traumatic stress disorder (PTSD). |
